This is the compulsory questionnaire for anyone who wants to train with Perth Triathlon Club.

The short questionnaire asks for medical background and personal information aimed at improving your experience with PTC

Depending on your responses, you may be required to seek medical clearance before commencing training with PTC.

All responses will be confidential and only supplied to those directly providing coaching services to the club.
