FSAT MEMBERSHIP APPLICATION/RENEWAL 2022

Membership Fees:

• TIER 1 - FULL MEMBERSHIP $95.00 This is for skating/competitive skaters 18 years and over. Includes $25 ISA Insurance fee.
• TIER 2 - JUNIOR MEMBERSHIP $95.00 This is for skating/competitive skaters under 18 years. Includes $25 ISA insurance.
• TIER 3 - AUSSIE SKATE LEVEL MEMBERSHIP $70.00 This is for skaters currently competing in Aussie Skate levels ONLY. You MUST be a current member of Aussie Skate.  (Does not include $25 ISA Insurance Fee as this is included in your Aussie Skate membership).
• TIER 4 - COACHING MEMBERSHIP $40.00 This is for non-competitive coaches only. You must have a current fully accredited APSA Membership. Does not include ISA Insurance.
• TIER 5 - FAMILY MEMBERSHIP (FULL) $88.00 FSAT skating/competitive members 18 years and over. A 10% discount entry for immediate family members living in the same household. Must have 2 or more family members of the same family. Includes $25 ISA Insurance.
• TIER 6 - FAMILY MEMBERSHIP (JUNIOR) $88.00 FSAT skating/competitive skaters under 18 years. A 10% discount entry for immediate family members living in the same household. Must have 2 or more family members of the same family. Includes $25 ISA Insurance.
• TIER 7 - FAMILY MEMBERSHIP (AUSSIE SKATE) $63.00 This membership is for Aussie Skate level skaters. You must be a current member of Aussie Skate. A 10% discount entry for immediate family members living in the same household. Must have 2 or more family members of the same family. Does not include ISA Insurance.
• TIER 8 - TEMPORARILY UNAVAILABLE 
• TIER 9 - ASSOCIATE MEMBERSHIP $0.00 This is a non-skating membership for Honorary Committee Members, Life Members, non-skating members, etc. Does not include ISA Insurance. 
• TIER 10 - TICKET TO PLAY You must have a current Government Ticket to Play voucher. If this applies to you, please contact FSAT prior to registering for this FSAT Membership.

*Notes:

1. Please enter the member's email address as one which you would prefer to receive all communication from FSAT (i.e. that of a parent/guardian if skater is under 18). Email is FSAT's primary form of communication.
2. Forms must be completed in the member's name (ie the skater's name even if they are under 18).
3. If you are the parent/guardian of a member under 18 years you are a member by proxy, and will need to enter your details, eg mobile phone number and email address, and must also complete the waiver as part of this registration.
4. FAMILY memberships MUST be from the same immediate family, one form must be completed PER family member.
5. NEW members joining from April onwards must select INTERIM/PRO-RATA Membership, and then add the pro-rata $7 for each month remaining of the calendar year in the Optional Items section of page 3.
