Instructions

  1. Each delegate needs to complete an individual registration form.
  2. Payment to be made by credit card.  C&K does not accept AMEX.
  3. Please Note:  Cut off registration date - 11 April 2011. 
  4. C&K reserves the right to alter this program or presenter without further notice, however the program is intended to run as advertised.
  5. C&K reserves the right to cancel a conference due to insufficient numbers - delegates will be notified if this occurs and payment refunded in full.
  6. Delegates will be liable for payment in the event of non-attendance unless cancellation or request for transfer is made in accordance with the cancellation policy - 7.
  7. Delegates can cancel their place by advising C&K in writing 3 weeks before the conference date. A cancellation fee of $25 will be retained. Delegates who wish to send a substitute in their place must advise C&K in writing.
  8. Exceptional circumstances will be assessed individually after the conference.
  9. Where an individual registration is for less than $1,000 (inc GST) this document becomes a tax invoice for GST purposes upon payment. A receipt will be issued via Register Now.
